Your medical record is strictly private. According to the Health Insurance Portability and Accountability Act (HIPAA) all patients will be given a Joint Notice of Privacy Practice. This Notice will outline how your medical information may be used and disclosed and how you may gain access to this information.

If you or any other individual desire a copy of your medical record, a signed authorization by you (or legal guardian) must be on file prior to the release of your records. Please make arrangements with our staff if you would like a family member or any other individual to have access to your private records.
